Job Title: Peer Support Specialist and Recovery Navigator**
Location: Hyannis/Falmouth, MA Job Type:
Part-time $23/hour

We are seeking a passionate and empathetic Peer Support Specialist and Recovery Navigator to join our team and make a meaningful impact on the lives of our patients.
Job Summary: As a Peer Support Specialist and Recovery Navigator, you will play a vital role in supporting patients who are struggling with substance use disorders. You will work closely with individuals recently visiting the emergency room, providing them with the necessary resources and support to facilitate their recovery journey. Your primary responsibilities will include offering peer support, assisting with the coordination of care, and helping patients find appropriate placements for detoxification or sober living environments.
Key Responsibilities: 1. Peer Support: - Establish trusting and supportive relationships with patients based on shared experiences and empathy. - Provide emotional support, encouragement, and motivation to individuals in recovery. - Share personal recovery experiences when appropriate to inspire hope and resilience.2. Resource Navigation: - Assist patients in identifying and accessing community resources, treatment programs, and support services. - Collaborate with healthcare providers, social workers, and community organizations to coordinate care and ensure seamless transitions. - Help patients find suitable placements for detoxification, residential treatment, or sober living environments.3.
Follow-Up Support: - Conduct follow-up meetings with patients to monitor their progress and address any challenges they may face. - Develop individualized recovery plans tailored to each patient's needs and goals. - Advocate for patients to ensure they receive the necessary support and resources for successful recovery.4. Education and Advocacy: - Educate patients and their families about substance use disorders, recovery processes, and available resources. - Advocate for patient needs and rights within the healthcare system and community.5.
Documentation and Reporting:** - Maintain accurate and confidential records of patient interactions and progress. - Prepare reports and documentation as required by the organization.*
*Qualifications:**- Lived experience with substance use disorder and a strong commitment to personal recovery.- Certification as a Peer Support Specialist or willingness to obtain certification within 6 months.- Excellent communication and interpersonal skills.- Ability to work collaboratively with a diverse team of healthcare professionals.- Strong organizational skills and the ability to manage multiple tasks simultaneously.- Empathy, patience, and a non-judgmental attitude towards individuals in recovery.**
